How to register a Cause Classification in Qualiex Occurrences?

The Cause Classification aims to identify and group the reasons or factors that led to the occurrence of an event. By categorizing a cause, it becomes possible to consolidate data and generate strategic insights to support decision‑making. This facilitates the detection of patterns, the identification of trends, and the implementation of improvements that help prevent similar situations from recurring.

This categorization simplifies the analysis and guides the adoption of appropriate corrective and preventive measures.

When the root cause — or a potential root cause — is identified through analysis tools such as the Ishikawa Diagram, the 5 Whys, or the Tree Diagram, it can be recorded and classified in the system through the Cause Classification feature, ensuring a structured and consistent treatment process.

1. Access the module

2. Registering a cause classification

To register a new classification, click the “Cause Classification” tab, then click , as shown in the image below.

To register a classification, you must have the permission “Manage types, categories, and cause classifications”, defined under Settings > Roles and Permissions.

A registration window will open where you must define:
 
  • Title (required):

    Enter the name of the classification (required; maximum of 150 characters).

  • Description (optional):

    Here you may describe the characteristics of this classification (optional; maximum of 1000 characters).

     

After clicking “Save”, the cause classification will be registered, and a confirmation message will be displayed:
